Abstract

Introduction: Apixaban, a direct factor Xa inhibitor used in the treatment of venous thromboembolism (VTE) and the prevention of recurrent events, is currently not covered by Vietnam's health insurance.

Objective: To analyze the budget impact of apixaban in the treatment of VTE and prevention of recurrent events in Vietnam.

Methods: A modeling study was designed, integrating a literature review and expert consultations to evaluate the budget impact (BI) of two hypothetical scenarios: the current scenario (maintaining treatment therapy) and a future scenario (adding apixaban to the health insurance coverage plan) over a five-year period. The analysis included drug costs, disease management costs, and event-related costs.

Results: Increasing the use of apixaban from 5.1% to 25.1% resulted in the prevention of 722 cases of events and a total five-year budget savings of VND 2.83 billion. These findings suggest that apixaban should be considered for the health insurance coverage plan.

Conclusion: The budget savings and reduction in adverse events associated with the treatment of VTE indicate that apixaban should be considered for appropriate reimbursement rates and conditions in the addition of the health insurance coverage plan.
