Mục tiêu: Khảo sát đáp ứng của ung thư trực tràng sau hóa xạ trị tiền phẫu. Đánh giá kết quả của phẫu thuật nội soi cắt trực tràng sau hóa xạ trị tiền phẫu.
Phương pháp: Hồi cứu loạt ca.
Kết quả: Có 284 BN (nam/ nữ là 1,25:1), tuổi trung bình 58,13 ± 11,305. Bướu trực tràng giữa dưới 251(87,2%), thể sùi 261(91,9%). Đáp ứng bướu giảm đáng kể giai đoạn bệnh và tình trạng di căn hạch (p < 0,001) pT0 (72/284), pN0 (231/284), đáp ứng pCR của nCRT (23,6%) và TNT (26,5%). Thời gian mổ 130±37,54 phút, bảo tồn cơ thắt (bướu <5cm 54,1%), rò miệng nối 14,4%. Không trường hợp tử vong.
Kết luận: Hóa xạ trị tiền phẫu làm giảm đáng kể giai đoạn bệnh, kích thước khối bướu, và tình trạng di căn hạch. Tỉ lệ đạt pCR, nhóm bệnh nhân được điều trị bằng phác đồ TNT cho thấy xu hướng đáp ứng tốt hơn so với nCRT truyền thống. Đặt tỉ lệ bảo tồn cơ thắt, biến chứng trong phạm vi cho phép, an toàn.
Abstract
Objective: To evaluate tumor response in rectal cancer following preoperative chemoradiotherapy and to assess the outcomes of laparoscopic rectal resection after preoperative chemoradiotherapy.
Methods: A retrospective case series.
Results: The study included 284 patients (male/female ratio 1.25:1), with a mean age of 58.13 ± 11.305 years. Mid to low rectal tumors accounted for 251 cases (87.2%), and the exophytic growth pattern was observed in 261 cases (91.9%). Treatment resulted in significant tumor downstaging and reduction in lymph node metastasis (p < 0.001). Pathological stages included pT0 in 72/284 patients and pN0 in 231/284 patients. The pathological complete response (pCR) rate was 23.6% with standard neoadjuvant chemoradiotherapy (nCRT) and 26.5% with total neoadjuvant therapy (TNT). Mean operative time was 130 ± 37.54 minutes. Sphincter preservation was achieved in 54.1% of patients with tumors <5 cm from the anal verge. The anastomotic leak rate was 14.4%. There were no postoperative mortalities.
Conclusion: Preoperative chemoradiotherapy significantly reduces tumor stage, size, and lymph node involvement. The pCR rate was favorable, with a trend toward better response in patients receiving the TNT regimen compared to traditional nCRT. Sphincter preservation rates and complication profiles were within acceptable limits, indicating the safety of this approach.
