Abstract
Mutations in the IGHMBP2 (immunoglobulin helicase μ-binding protein 2) gene are associated with distal spinal muscular atrophy type 1 (DSMA1) and Charcot-Marie-Tooth disease type 2S (CMT2S) in both pediatric and adult patients. Although rare, IGHMBP2 mutations have been documented in the Vietnamese population. In addition to increasing infant mortality, these mutations may cause slowly progressive psychomotor developmental delay, resulting in a substantial socioeconomic burden on affected families and society. The management of disease-causing genetic variants is therefore of considerable importance, particularly in the context of Vietnam’s impending population aging, the implementation of policies encouraging population growth and the rapid expansion of assisted reproductive technologies. We report the case of a 6-year-old boy diagnosed with CMT2S caused by an IGHMBP2 mutation. The patient was referred for evaluation of the underlying genetic etiology and genetic counseling. The child developed symptoms at 4 months of age, including psychomotor developmental delay, distal muscle atrophy, spinal deformity, and preserved nerve conduction velocity. Electromyography findings were suggestive of CMT2S. Routine genetic testing for common pathogenic variants yielded negative results. Whole-genome sequencing (WGS) identified a c.1334A>C (His445Pro) variant in the IGHMBP2 gene. Family segregation analysis demonstrated that the variant was inherited from both parents. Establishing a definitive molecular diagnosis remains a significant challenge in clinical practice in Vietnam. Greater emphasis should be placed on the management of disease-causing genetic variants within the Vietnamese population, particularly during a period characterized by pro-natalist policies and the growing use of assisted reproductive technologies. Accurate genetic diagnosis of hereditary neurological disorders and effective management of pathogenic variants play an essential role in promoting healthier future generations.
