Abstract
Introduction: Cystitis caused by ketamine abuse was first reported in the medical literature in 2007. Recently, a lot of young people in many countries have commonly used addictive drugs made from ketamine in entertainment clubs (also known as "ecstasy", "keken"). After a long time of using ketamine, many of them have symptoms similar to cystitis, which are often very severe and seriously affecting their quality of life. Treatment is usually difficult and often leaves long-term sequelae. This article aims to investigate the medical literature on the history of disease detection, pathogenesis, histopathology, clinical manifestations, diagnosis and treatment of ketamine-induced cystitis.
Materials & Methods: Search Pubmed literature from 1 January 2007 to 1 March 2025, with the keywords ketamine cystitis, ketamine-induced cystitis, ketamine-related cystitis, ketamine uropathy, and select overview, review, systemic review articles. The reason started in January 2007, because this was the time when there was the first report in the medical literature on ketamine-induced cystitis.
Results: A total of 39 papers were classified as overviews, but after reviewing, we selected 20 suitable articles. The articles were not selected because they do not primarily report on ketamine-induced cystitis but are only part of the chronic cystitis, or mention ketamine as one of the narcotic substances. We also added a number of clinical case series reports with medical overview, systematic review. The following are general comments about ketamine-induced cystitis: Clinical symptoms: Presentation of lower urinary tract symptoms and bladder pain in people with ketamine addiction; Some patients may have hydronephrosis. Imaging: Many patients have recorded a thick-walled bladder, small capacity; Some patients may have hydronephrosis. Cystoscopy: ulcerative inflammation with bleeding mucous membranes can be seen. The pathogenesis of ketamine-induced cystitis is complex and involves various pathways, among which the toxicity of ketamine along with its metabolites to the urothelium stands out. Histopathology: characterized by the urothelium being infiltrated by many inflammatory cells such as mast cell and eosinophils. Diagnosis: based on careful medical history, cystoscopy, and histology. Treatment: the first step is to tell the patient to stop using ketamine. Medical treatment can help relieve symptoms if the patient has not been on ketamine for a long time and does not reuse it. The longer ketamine is used, the less effective medical treatment will be. Some studies show that instillating hyaluronic acid into the bladder or injecting botulinum toxin type A into the bladder wall are effective in relieving symptoms for some patients. In cases where irreversible bladder variants cause bladder contractility, intestinal cystoplasty should be considered to increase bladder capacity and and improve kidney function.
Conclusion: Ketamine abuse and addiction are becoming more and more widespread, leading to the detection of more and more ketamine-induced cystitis. The symptoms are sometimes very severe, seriously affecting the quality of life. Over time, it will cause irreversible bladder deformity, making treatment very difficult. Therefore, ketamine-induced cystitis can be considered a new challenge for urologists
