Abstract
Background: Effective postoperative pain management is a cornerstone of modern surgical care. Appropriate selection and rational use of analgesics play a critical role in improving patient comfort, accelerating recovery, and optimizing treatment outcomes. However, data on postoperative analgesic utilization and prescribing practices in private hospitals in Vietnam are still limited.
Objectives: To describe the utilization pattern of postoperative analgesics and to assess the appropriateness of analgesic prescribing at Van Hanh General Hospital in 2024.
Methods: A descriptive cross-sectional study was conducted based on medical records of patients who underwent surgery and received postoperative treatment at Van Hanh General Hospital in 2024. Data collected included patient demographics, surgical characteristics, types of analgesics, routes of administration, combination regimens, and prescription appropriateness. Appropriateness was evaluated according to the Vietnamese National Drug Formulary (2022) and manufacturers’ prescribing information, focusing on indication, single dose, and dosing frequency. [1]
Results: A total of 350 medical records were analyzed. Female patients accounted for 84.3% of the total, and regarding the age distribution, 72.6% of all patients were aged 18-39 years. Reconstructive surgery was the most common procedure (80.9%). Paracetamol was the most frequently prescribed analgesic (43.9%), followed by ketorolac (34.5%). Intravenous administration predominated (75.7%). Combination analgesic therapy was prescribed in 65.0% of cases, most commonly paracetamol combined with ketorolac (53.9%). Overall, inappropriate prescribing was uncommon, with 0.3% of prescriptions inappropriate regarding indication, 0.3% regarding single dose, and 1.2% regarding dosing frequency. A total of 98.8% of prescriptions met all three appropriateness criteria.
Conclusions: Although the use of analgesics has largely met the pain management needs of patients, some cases of inappropriate prescribing still remain. Therefore, it is essential to further improve prescribing appropriateness by standardizing treatment protocols and enhancing the supervisory role of clinical pharmacists.
