Evaluation of the correlation between serum ferritin concentrations and metabolic and inflammatory risk factors in prediabetic patients

Objective: To evaluate the correlation between serum ferritin levels and various factors including age, BMI, waist circumference, blood glucose levels, HbA1c, lipid profile, hs-CRP, and TyG index. Methodology: A cross-sectional descriptive study was conducted on 84 patients aged ≥ 35 years diagnosed with prediabetes and receiving treatment at the Internal Medicine A Department, Hue Central Hospital, Facility 2, from June 2023 to August 2024. All patients meeting the inclusion criteria were enrolled. The oral glucose tolerance test (OGTT) was performed to determine fasting glucose (G0) and 2-hour plasma glucose (G2). Laboratory tests included HbA1c, serum ferritin, hs-CRP, and lipid profile. Data were processed using SPSS 25.0. Results: Serum ferritin level showed a moderate positive correlation with BMI (r= 0.490, p < 0.001), waist circumference (r= 0.355, p < 0.01), triglycerides (r = 0.352, p < 0.01), hs-CRP (r=0.306, p < 0.01), and the TyG index (r = 0.360, p < 0.01). A moderate inverse correlation was found between ferritin and HDL-C (r = –0.304, p < 0.01). Multiple regression analysis revealed correlations between ferritin and BMI, HDL-C, and hs-CRP, with the regression equation: Ferritin = 0.479 × BMI – 0.193 × HDL-C + 0.231 × hs-CRP with r = 0.599, R² = 0.359, and model significance p <0.001. Conclusion: Serum ferritin levels in patients with prediabetes showed moderate positive correlations with metabolic and inflammatory risk factors, particularly BMI, waist circumference, triglycerides, hs-CRP, and the TyG index, and an inverse correlation with HDL-C. Multivariable analysis identified BMI, HDL-C, and hs-CRP as independent factors associated with serum ferritin levels, suggesting that ferritin may serve as a biomarker reflecting metabolic–inflammatory disturbances in patients with prediabetes.
