Clinical characteristics and Rheoencephalographic parameters in patients with type 2 diabetes mellitus at Hue Central Hospital

Objective: To investigate selected clinical characteristics, rheoencephalographic parameters and the relationships between rheoencephalographic parameters and age, disease duration in patients with type 2 diabetes mellitus at Hue Central Hospital. Methodology: A cross-sectional descriptive study was conducted on 50 patients with type 2 diabetes mellitus (T2DM) who were hospitalized at the Department of General Internal Medicine – Geriatrics, Hue Central Hospital, from September 20, 2023, to November 10, 2023. Data were collected and analyzed using Microsoft Excel and IBM SPSS version 20.0. Results: In patients with type 2 diabetes mellitus with the mean age of patients was 70.66 ± 11.88 years, the duration since diagnosis of type 2 diabetes mellitus of< 5 years accounted for the highest proportion (42%), followed by 5–10 years (30%), while a duration of ≥ 10 years accounted for 28%. The predominant cerebral hemodynamic waveform morphology was the blunted peak. Secondary (dicrotic) waves appeared infrequently and were mostly blurred in both the right and left hemispheres on both mastoid–occipital and frontal–mastoid leads. In the right hemisphere, increased vascular tone (VT) was 68% on the mastoid–occipital lead, higher than 60% on the frontal–mastoid lead (p = 0.026). Reduced blood flow intensity (BFI) was 38% on the frontal–mastoid lead, higher than 30% on the mastoid–occipital lead, with a statistically significant difference (p = 0.036). In the left hemisphere on the frontal–mastoid lead, increased VT was more common in patients aged ≥ 60 years (76.2%) compared with those aged < 60 years (35.7%) (p = 0.043). On the frontal–mastoid lead in the right cerebral hemisphere, the proportion of patients with reduced BFI was higher in the < 60-year age group (75%) compared with the ≥ 60-year group (31%) (p = 0.041). No association was observed between increased or reduced BFI in either cerebral hemisphere on both lead and the duration since diagnosis of type 2 diabetes mellitus. Conclusions: In patients with type 2 diabetes, rheoencephalographic abnormalities were common, characterized mainly by blunted waveforms and increased vascular tone. Age influenced vascular tone and blood flow intensity while the duration of diabetes was not associated with cerebral hemodynamic changes.
