The golden window for treating basedow eye disease and warning signs

Background: Thyroid Eye Disease (TED) affects 13–69% of Graves’ patients, mainly in the hyperthyroid phase (90%) but also in euthyroid or hypothyroid phases (10%). The golden window (6–24 months) is crucial for preventing progression. Key points: Active Phase (6–24 months): Inflammation,  exophthalmos,  vision risk. Stable Phase: Symptoms persist but inflammation subsides. Early treatment (CAS ≥ 3) is essential: Smoking cessation, Selenium, NSAIDs, corticosteroids, immunosuppressants, Ophthalmologic management. Warning: TED symptoms and positive TRAb/TSI in euthyroid patients may predict Graves’ disease onset. Conclusion:  Early  intervention  within 24 months prevents severe outcomes. Monitoring elevated TRAb/TSI in euthyroid patients is key to early Graves’ diagnosis.
