Abstract

Background: The effectiveness of endoscopic clipping and continuous high-dose proton pump inhibitors (PPIs) has been well-documented. Thanh Hóa Provincial General Hospital has applied a combined treatment approach using hemoclips for hemostasis in endoscopic treatment of high-risk bleeding gastric and duodenal ulcers along with high-dose continuous PPI therapy, achieving notable outcomes. Objective: (1) Evaluation of the clinical and paraclinical symptoms of gastrointestinal bleeding caused by peptic ulcers at Thanh Hóa Provincial General Hospital in 2024. (2) Evaluation of hemostasis outcomes using clips combined with Esomeprazole in patients with gastrointestinal bleeding due to peptic ulcers. Subjects and Methods: 33 patients with bleeding peptic ulcers classified as Forrest grade IIB or higher were included in the study. Hemoclips from Olympus were used. Esomeprazole was administered as an 80 mg bolus injection followed by a continuous intravenous infusion of 8 mg per hour for 72 hours.Results: Immediate hemostasis was achieved in 32 out of 33 patients (97%). Rebleeding occurred in 9.1% of patients. The overall treatment success rate was 97%. The average length of hospital stay was 7.61 ± 3.50 days. Conclusion: The combination of hemoclips with intravenous proton pump inhibitors proves to be effective and safe for patients with bleeding peptic ulcers.
